REGISTRATION

ONLINE REGISTRATION:
Registration for the 2017 Summer Recreation Program will be conducted via the Internet (online application) Online Registration instructions can be found on one of the links under Summer Recreation.  

A family information form, medical form, and photo release form must be completed for each camper. You will receive these forms via email two weeks prior to the start of camp along with important information regarding the Summer Program.  All registrations will be processed on a first-come, first-served basis. A 3.5% fee will be charged to you for every transaction that is made via payschools if you are seeking a refund.

MAIL-IN REGISTRATION
In order to ensure a place in any of the programs (half or full-day) 'MAIL IN' registration forms and payment for the full tuition must be received when you register. If you are registering your child for Arts & Crafts, Advanced Arts & Crafts, Cooking, Science or Sculpture, Music Lessons you must pay a $10 materials fee for each course at time of registration. Please make checks payable to Hicksville UFSD.

GENERAL INFORMATION

Nurse -- A nurse will be assigned daily to the middle school from 8:30 a.m. until noon. A second nurse will be assigned from 11:45 a.m. to 3:30 p.m. There will be two nurses at the camp in the afternoon. A nurse always attends field trips.

Physical Examinations -- Physical examinations are required for all program participants. The medical form enclosed in this brochure must be completed by your physician indicating that your child has received an annual physical and the required doses of immunization.

All forms must be returned no later than June 7, 2016. Attached is the required medical form to be used by all campers.
 
TUITION COSTS:
  • Half Day Fees:
    • 1 Session:    $150
    • 2 Sessions:  $300
    • 3 Sessions:  $450
  • Full Day Fees:  $790.00
  • Students who attend any other district program in the morning session who wish to attend the
    afternoon session (Full Day-6 Weeks) will be charged $500.00.
  • Please note that there will be no refunds will be made after May 1st.
