Sports Physical Dates

The School District provides Seasonal Sports Physical Exams to those students who require one prior to the start of the respective season, at no cost. These are for Athletic Participation Examinations only.

PHYSICAL EXAMINATION FOR ATHLETICS:*The Sports Physical is valid for one full year.

It is required that each student athlete submit a sports physical, medical questionnaire and parental consent form in order to participate in interscholastic athletics. The forms can be obtained in the Athletic Office (Rm. 157) or the nurses office or online. All completed forms must be handed into the Nurse's office.
Students must use the District Medical/Physical forms only!

In addition to the medical, it is the student's responsibility to complete a parental consent form and Concussion Form. These forms must be completed no fewer than five days prior to involvement in a sport.

Sports Medicals will be administered by the District Physician, Dr. Karl Friedman on the following dates at no cost

MS Nurses Office
Please make sure that your child sees the nurse prior to that date should they need a physical exam.

Nurses will be in on the fpollowing dates over the summer shuld you need to drop off any paperwork.

HIGH SCHOOL

HS FALL SPORTS PHYSICALS

HS Nurses Office
Nurses will be in over the Summer from 8am-2:30 pm on the following dates. If you would like to drop off your paprework.

8/17, 8/20, 8/21, 8/22, 8/23

8/24, 8/27, 8/28, 8/29, 8/31
