Home Test Pick-up

If anyone needs COVID-19 test kits, tests are available at your child's school buildings during school hours. You may go to the front doors to request kits.

You may also call the Office of Public Information at 516-733-2189 M-F between the hours of 8 AM to 4 PM for further assistance.

Testing Facility or Doctor's Office Student Reporting

When reporting a positive test result from an outside testing facility or doctor's office, please: 

1. Upload the positive result confirmation notice you receive from the facility or doctor's office.
2. Fill out the Student COVID Test Reporting Form
3. Our COVID Coordinator will notify the Nassau County Department of Health


Home Test Student Reporting

Prior to uploading results from home test kits, please: 

1. Write the student's name and date of test on the outside of the test kit.
2. Take a picture of the test results for upload.
3. Fill out the Student COVID Test Reporting Form
4. Our COVID Coordinator will notify the Nassau County Department of Health
