MODIFIED (MIDDLE SCHOOL) PROGRAM - 7th & 8th GRADE

IMPORTANT INFORMATION REGARDING
PHYSICAL EXAMINATION FOR ATHLETIC PARTICIPATION for 2020-2021
NYSED waived the physical exam requirement for students for the 2020-21 school year. if they have previously had a health examination for school attendance in the 2018-2019 or 2019-2020 school years and provide a current health history completed and signed by their parent/guardian within 30 days of the start of the season, unless a current health examination is deemed necessary by the school district’s director of school health services (medical director).
***Athletes that have tested positive for Covid 19 infection require a letter from doctor stating they have recovered from Covid 19 infection and can "participate in sports without restriction"

MIDDLE SCHOOL FALL SPORTS
**STARTS MARCH 22nd**
FOOTBALL / B&G SOCCER / B&G CROSS COUNTRY
Please read the information provided below
Students must provide the 'Interval Health/Parental Consent' Document to the nurse prior to tryouts. (Can be found on Athletics Website - "Forms for Participation"
All Students will be masked everywhere we go. When the game/competition starts we encourage all student athletes to wear a mask. If they cannot 'tolerate' playing with a mask then they may take it off during game play/competition only (as per NYS DOH/NC DOH) .
TRYOUTS
All Students from all Cohorts / both remote and hybrid will tryout on the same days-
Tryouts start on March 22nd at 3:00pm
**Football will be held on the grass field (next to the MS Turf)
**Boys & Girls Soccer will meet on the MS Turf
**Boys & Girls Cross Country will meet in the MS Main Gymnasium on the bleachers
REMOTE STUDENT ENTRY: If you are entering the building (Remote Students) please enter through the 4th Street Entrance near gymnasium.
SCREENING: Temperature Screening will take place prior to participation.
**Practices schedules (dates) will vary each week due to home games.
LOCKER ROOM USE: There will be limited access to locker rooms for changing only. Football Players will store their equipment in the locker rooms and be able to enter 8 at time to dress. Students must wear a mask at all times. Students may remove the mask during game play IF it becomes "intolerable" (as per NYS and NC DOH)
WATER: Please supply your child with a water bottle- none will be provided on site.
SPECTATORS: We will permit '2' parents for each child at Athletics Events held on the Hicksville Middle School Campus- Outdoor Fields. Parents willreceive a badge with theirchild's name on it and the sports played which must be presented at all home events at the entry gate(which will be designated prior to the start of the season). No Badge - No Entry- Attendance will be taken at the gate- Contact Tracing reasons and maintaining security)
At this time there will be no spectators at Cross Country Events. (This is a County wide Decision - May change)
TRANSPORTATION: There will be a 4:40 late bus available only. If parents are picking students up they must be picked up promptly at 4:40 promptly.

PLEASE GET THIS FORM THE THE NURSES OFFICE PRIOR TO MARCH 1ST.
