Medical & Immunization Requirements
Student Physicals
According to Education Law Section 903, every student in Pre-K, Kindergarten, 1st, 3rd, 5th, 7th, 9th, and 11th grades and all new admissions to the district are required to submit a health certificate signed by a duly licensed physician authorized to practice medicine in New York State.
The physical examination form will request a body mass index (BMI) and weight status category. NYS requires biennial reporting of weight status categories. To opt out of weight status reporting, please contact your school nurse.
A notice of request for dental health certificates shall be distributed at the same time that parents/guardians are notified of health examination requirements. The notice shall include a statement that a list of dental hygienists to which students who need comprehensive dental assessments may seek dental care on a free or reduced cost basis is available upon request.
Whenever possible, physical examinations should be conducted by your family physician. Physicals must be dated no more than 12 months prior to the first day of school the previous year. If we do not receive the information from you or your family physician, your school nurse will contact you to remind you of the need to provide the physical form. Please be advised that the school nurse will send a final notification that will inform you that you must comply with this requirement or the district physician will perform the medical examination. The physical must be on the required health examination form.

All schools are obliged to enforce the requirements of New York State Public Health Law, Section 2164. As of July 1, 2015,
no child shall be admitted to school, or in the case of students entering from outside New York, be allowed attend school,
in excess of 14 days without satisfactory written evidence that the student has been immunized. Below is a summary of the School Immunization Requirements for the 2021-2022 school year based on NYSDOH amended regulations:

On June 13, 2019, Governor Andrew M. Cuomo signed legislation removing non-medical exemptions from school vaccination requirements for children.

Medical Exemptions
Medical exemptions must be renewed annually; it must contain
information to identify medical contraindications to specific
immunization, must specify the length of time immunization
contraindicated and must be on the NYS Department of Health Medical Exemption Form.
