Everything families need before participating in athletics.

Hicksville Public Schools uses FamilyID for online athletic registration. FamilyID provides a secure and convenient way to register for sports, eliminating the need to submit paper forms to the school nurse or athletic office.
Once you create your account, your information is saved, making it easy to register for future sports seasons or multiple student-athletes.
Before You Register
To participate in a sport, student-athletes must have a current physical examination completed within one year of the start of the sports season.
Required Documents
Please upload the following documents during the Participant Information section of your FamilyID registration:
- Current Physical Examination
- Interval Health Sports Update Form

Please download the necessary forms you require located below and return to the Coach and Nurse (as specified).
- The Physical Examination, Parental Permission & Concussion Forms are required for participation on any Athletic Team at every level (MS & HS).
- Please make every effort to have the permission/medical forms to the nurses prior to the start date for each sport so you may start practice immediately.
- Physical Examination: Must be completed on the form provided by the district (below). They are valid for one full year. Please bring to Nurses Office.

Free School Sports Physical
Date: Monday, August 17, 2026
Time: 8:30 a.m.
Location: Hicksville High School Nurse's OfficeThe district offers free sports physicals for students who need one before the start of a sports season. Physicals are provided by Dr. Karl Friedman, District Chief Medical Officer, and are for athletic participation only.
Before Your Appointment
Before attending a school sports physical, please:
- Complete the Interval Health History Form in FamilyID.
- Upload all required forms to FamilyID.
- Use the New York State Physical Examination Form available under Required Forms for Participation on the Athletics website.
Required for Athletic Participation
To participate in interscholastic athletics, students must complete:
- Current Sports Physical (valid for one year)
- Interval Health History Form
- Parent/Guardian Consent
- Concussion Information Form
All required forms must be completed and submitted through FamilyID before participating in a sport.
Note: Families are encouraged to obtain a physical from their own healthcare provider whenever possible.

The Athletic Placement Process is for 7th and 8th-grade students who demonstrate the ability to compete a
the junior varsity (JV) or varsity level.Eligibility
To be considered for the APP, students must:
- Be recommended by a varsity coach.
- Be invited to try out based on demonstrated skill.
- Complete all required medical and fitness evaluations.
Please Note: Modified students who participate at the high school level through the APP are not eligible to receive the Middle School "4" Sport Award.
The APP Process
1. Coach Recommendation
A varsity coach must recommend the student and invite them to participate in the APP.2. Parent Request
A parent or guardian submits the required APP request form to the Athletic Office.3. Medical Clearance
Students must have a current physical examination, including the required maturation assessment (Tanner Level), before continuing in the process.4. Physical Fitness Assessment
Students must successfully complete the New York State APP fitness assessment, administered by a certified physical education teacher. Most students must score at or above the required standard in 4 of the 5 fitness tests. (Fitness testing is not required for bowling or golf.)Testing Schedule
APP testing is conducted before each fall, winter, and spring sports season. Students will be scheduled for testing after all required paperwork has been received and medical clearance has been approved.

Participation in athletics and extracurricular activities is a privilege. Student-athletes are expected to maintain satisfactory academic progress while participating in school programs.
Eligibility Requirements
- Students who are failing two or more classes are not eligible to participate in athletics or extracurricular activities unless they are placed on academic probation.
- Students failing one class will receive a notification that they are at risk of probation if their academic performance declines.
- Students on academic probation must attend all required extra help sessions and meet the expectations established by the school.
- Failure to attend required extra help sessions will result in loss of eligibility.
- Academic eligibility is reviewed at the middle and end of each marking period, as well as after final grades and summer school when applicable.
Students and parents will be notified in writing if a student is placed on academic probation, becomes ineligible, or regains eligibility.
